Endorsement for Change in Term Dates

Description
Anytime a policy has the start date modified, we need to tell the underwriters via endorsement page.
The declarations page should reflect the new dates, and the endorsement page should notate the original dates, and the new dates of coverage.

Hi Leo,
You can re-create this edit. When you run your test purchase, ensure the entry date is in the future. IN the example belote, it is December 31,2024.
(1) The date is moved back When running a 'basic edit'. The length of the policy should never change
(2) There is an issue with this form. The state is not being pulled
