Sprint 3 - PaisanoAssist

Information needed to issue an assist package are as follows:

*Purchaser First Name
*Purchaser Last Name
*US Street Address
*US Street Address 2
*US City
*US State
*US Zip
*US Phone Number
*Dependent Names (number of fields rendered based on input from step 1, one field for first and last name - no need to separate into different fields)
*Main Contact in Mexico (should be spouse, or common law spouse)
*Address in Mexico

• This will require some additional programming and database entries. Currently in V3 for MXHO, the postal code is used to populate the State, City, and Colonia (neighborhood). We can migrate the DB information over to V4 from V3, but programming for the queries will need to be implemented. This will need to be accomplished with AJAX or something similar to populate on the same page as the postal code entry.
